Cherished Hands Home Healthcare LLC Terms and Conditions

These Terms and Conditions (“Agreement”) govern the provision of home healthcare services by Cherished Hands Home Health LLC (“Company,” “we,” “us,” or “our”) to clients (“Client,” “you,” or “your”). By using our services, you agree to the following terms:

  1. Services Provided

Cherished Hands Home Health LLC offers non-medical and skilled home health services, including but not limited to:

  • Personal care (bathing, grooming, dressing) * Pediatric Care
  • Companion care and homemaking * Specialty Care
  • Medication reminders and management * 24Hours Care
  • Skilled nursing services * CPR Training
  • Developmental Disability Service * Staffing Services 
  • Respite care for family caregivers * CPR Training.

 

Service availability may vary based on location, client needs, and care plans.

  1. Text Messaging communications 

By opting in to receive text messages from us, you agree to the following terms:

  1. CONSENT & OPT-IN

By providing your phone number, you agree to receive text messages from Cherished Hands Home Health LLC, including but not limited to:

  • Appointment reminders
  • Schedule updates for caregivers
  • Billing and payment notifications
  • Health and wellness tips
  • Company updates and announcements

The information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

You may opt in by:

  • Signing up via our website form or patient intake forms
  • Replying START to a text message invitation
  • Giving verbal or written consent to our staff

  1. MESSAGE FREQUENCY
  • The number of messages you receive may vary based on your service plan, schedule, and communication preferences.
  • Clients and caregivers may receive 1-2 messages per week depending on appointment reminders, shift updates, or urgent notifications.

  1. OPT-OUT & UNSUBSCRIBE

You can opt out of text messages at any time by replying:

  • STOP – To unsubscribe from all text communications
  • HELP – To receive assistance or support regarding text messages

Once you opt out, you will no longer receive SMS messages but may still receive emails or calls for important service updates.

 

  1. MESSAGE & DATA RATES
  • Standard message and data rates may apply depending on your mobile carrier.
  • Cherished Hands Home Health LLC is not responsible for any charges incurred by receiving text messages.
  • Contact your mobile provider for details on your messaging plan.
  1. Eligibility & Service Agreement

To receive services, the Client must:

  • Undergo an initial assessment by our healthcare team.
  • Provide accurate medical history, care needs, and emergency contacts.
  • Sign a formal Service Agreement outlining care details, fees, and schedules.
  • Consent to periodic re-evaluations to adjust care plans as necessary.
  1. Payment & Billing
  • Clients are responsible for timely payment of services unless covered by insurance or a third-party payer.
  • Payment methods accepted: (ACH, Credit/Debit Card, Check, Bank Transfer, Long-Term Care Insurance, Medicaid, etc.).
  • Invoices are issued on a weekly cycle
  • Late payments may result in service suspension or additional fees.

Cancellation Policy: Clients must provide [e.g., 24-48 hours] notice for service cancellations. Failure to do so may result in a cancellation fee.

  1. Insurance & Medicaid Coverage
  • If insurance covers services, the Client must provide all required documentation.
  • Clients are responsible for any co-pays, deductibles, or uncovered services.
  • Medicaid clients must comply with state regulations for eligibility.
  1. Client Responsibilities

Clients and/or their families must:

  • Provide a safe and clean environment for caregivers.
  • Treat caregivers with respect and dignity.
  • Notify Cherished Hands Home Health of any schedule changes, health updates, or hospitalizations.
  • Secure any pets that may pose a risk to caregivers.

Failure to uphold these responsibilities may result in service termination.

  1. Caregiver Conduct & Replacement

Our caregivers are trained, licensed (as applicable), and undergo background checks. They are expected to:

  • Provide professional, compassionate, and ethical care.
  • Maintain confidentiality of client information.
  • Follow the agreed-upon care plan.

Client Requests for Caregiver Changes: If a client is dissatisfied with a caregiver, they may request a replacement. We will accommodate requests based on availability.

  1. Confidentiality & HIPAA Compliance

Cherished Hands Home Health follows HIPAA regulations to protect client information.

  • We do not share personal or medical information without client consent.
  • Clients may request access to their records as permitted by law.
  1. Service Termination

Cherished Hands Home Health may terminate services if:

  • A client or family member is abusive or unsafe toward caregivers.
  • Payments are consistently late or unpaid.
  • Care needs exceed our scope of services.
  • The client voluntarily discontinues care.
  1. Liability & Indemnification
  • Cherished Hands Home Health is not liable for pre-existing health conditions or medical emergencies outside of the care provided.
  • Clients agree to hold Cherished Hands Home Health harmless from any damages caused by unreported health conditions, non-compliance with care plans, or third-party actions.
  1. Personal Identification Information Use: 
  • Our policy regarding collecting, using, and disclosing personal information is used to prove and improve our service. We will not use or share your information with anyone except as described in this Privacy Policy.
  • To improve your experience using our service, we may require you to provide us with certain personally identifiable information, including but not limited to your name, phone number, email address, and postal address. The information we collect will be used to contact or identify you.
  • By signing up and submitting your contact information (name, email, phone number), you can expect to receive messages regarding our services, appointment booking, and appointment reminders.

  1. Dispute Resolution
  • Any disputes must first be addressed through internal mediation.
  • If unresolved, disputes will be handled through the appropriate authority
  1. Amendments & Updates

Cherished Hands Home Health reserves the right to update these Terms and Conditions. 

Clients will be notified of significant changes.
